immunomodulatory peptide raises a handful of sensible questions. This page answers them in order, starting with the fundamentals and moving to applications.
This page was last updated on 2025-10-24 and is reviewed periodically as new material appears.
The molecule consists of 28 amino acid residues with an acetyl group attached to the N-terminal serine. Its sequence is acidic overall, with several glutamic and aspartic acid residues distributed along the chain and no cysteine, so disulfide bridges do not form. The peptide carries a net negative charge at physiological pH. Because the N-terminus is blocked, the intact molecule resists degradation by many aminopeptidases, which contributes to its stability in biological fluids.
The peptide is generated in cells by cleavage of prothymosin alpha, a larger acidic protein encoded by the PTMA gene. Prothymosin alpha is expressed in many tissues, not only in the thymus, and its functions include nuclear roles in chromatin-related processes. The 28-residue fragment corresponds to the N-terminal portion of that precursor. How the cleavage occurs and how the fragment's concentration is regulated remain open questions; circulating amounts are small and difficult to measure reliably with routine assays.
Overall evidence quality varies considerably. A large share of published reports come from single centers, rely on surrogate immunological markers, or lack adequate control groups. Systematic reviews have highlighted this heterogeneity as a barrier to pooling results. Open questions include which patients, if any, might benefit, what treatment duration is appropriate, and whether any effect is independent of standard care. The peptide is often described as an immune modulator rather than a therapy for one disease, which complicates confirmatory trial design.
Thymosin alpha 1 was identified in 1977 as a component of thymosin fraction 5, a heterogeneous preparation used in early studies of thymic function. Investigators purified the active material and determined its amino acid sequence, which enabled chemical synthesis. Work in the following decades concentrated on T-cell maturation and immune reconstitution in animals and small human cohorts. Early preparations varied in composition, so results from that period are difficult to compare with studies using defined synthetic peptide.
Clinical research has examined the peptide in chronic hepatitis B and C, as a vaccine adjuvant, and in sepsis and oncology settings. Findings across trials are mixed; some report changes in selected immune markers, while others find no clear clinical benefit. Many studies are small and define outcomes differently, which limits comparison. Regulatory approval is confined to a few countries, and the compound is not an approved drug in the United States or most of Europe.

Immunological studies connect the peptide to multiple parts of the immune response. It has been reported to engage Toll-like receptor signaling, to promote dendritic cell maturation, and to influence the balance of T helper cell subsets. Changes in natural killer cell activity and in cytokine release appear in cell culture and animal models. These observations describe broad immunomodulatory behavior rather than a single defined receptor target, and the primary molecular interaction has not been settled.
Thymosin alpha-1 is a synthetic 28-amino-acid peptide whose sequence was first identified in extracts of bovine thymus tissue during the 1970s. The chain carries an acetyl group on its N-terminal serine. Its acidic residue content is high, which produces strong water solubility and an isoelectric point well below neutrality. Material supplied for laboratory and clinical use is manufactured by solid-phase peptide synthesis rather than purified from animal tissue. Different salt forms, such as the acetate, alter the counter-ion content without changing the peptide backbone.
=== Cochlear implants === In parallel to the development of neuromodulation systems to address motor impairment, cochlear implants were the first neuromodulation system to reach a broad commercial stage to address a functional deficit; they provide sound perception in users who are hearing-impaired due to missing or damaged sensory cells (cilia) in the inner ear. The approach to electrical stimulation used in cochlear implants was soon modified by one manufacturer, Boston Scientific Corporation, for design of electrical leads to be used in spinal cord stimulation treatment of chronic pain conditions.

== Toxicity == Studies carried out on the toxicity of convulxin show that symptoms are a function of the dose. The effects of the toxin stand out through the sudden and brief duration of the symptoms after exposure. In mice, low doses (5 μg/animal) administered intravenously (I.V) elicited tachypnea, followed by apnea, within 20 seconds. The ED50 for brief duration apnea was determined to be 180 μg/kg. Higher doses (10 μg/animal) evoked intense convulsive crisis, and usually ended with the death of the animal. LD50 was determined to be 524 μg/kg. Intraperitoneal (I.P.) administration of up to 200 μg/animal proved to be ineffective. In cats, I.V. injections of 100 μg/kg showed respiratory disturbances, miosis, salivation, abdominal cramps, nystagmus, loss of equilibrium, convulsions and sometimes a brief phase of hypotonia. The ED50 dose for convulsions is 80 μg/kg. The majority of animals recovered within 30 minutes. In dogs, the effect of convulxin showed two stages. After I.V. injection of 100-125 μg/kg they became excited, barked and exhibited loss of equilibrium, respiratory disturbances, nystagmus, urination, defecation and vomiting. After recovery, two out of five animals had intermittent crisis of clonic convulsions that appeared after 24 hours and lasted until their death. The other three dogs showed periods of vivid agitation alternating with torpor. While the lethal dose in humans is not yet known, what has been discovered is that the level of toxic effects depends on the origin of the snake. As for a cure, so far a polyvalent snake antivenom is being used.
The staff union won 3.5 percent pay increases in 1998 and 1999 and lump-sum bonuses paid in 1996 and 1997, enhancements to the dental plan, and concessions on the prescription drug plan and seniority rights. Sheedy's re-election campaign was a difficult, and ultimately unsuccessful, one. Beginning in the spring of 1996, Roger Benson began putting together a statewide organization ("Members First") and publishing an alternative newsletter. Benson put together a full slate of officers and formally launched his campaign at PEF's October 1996 convention. Benson announced a four-part platform: 1) Establishment of a PEF department focused on protecting employment security; 2) Budgeting substantial resources to enforce the Civil Service merit system; 3) Employment of a full-time professional contract negotiator; and 4) Elimination of officer perks. A third candidate, Jim Israel (a PEF shop steward), entered the race in January 1997 but withdrew when he was unable to obtain enough signatures to secure his nomination. Benson's slate of candidates included Jane Hallum (a computer programmer) for secretary-treasurer and future PEF president Kenneth Brynien as an executive board member. Sheedy and Benson battled over the state pension surplus, and engaged in a letter-to-the-editor war in the newspapers. Benson promised not to raise dues and re-open union offices, while Sheedy admitted a dues hike might be necessary in 1999. In June 1997, Benson defeated Sheedy by a vote of 11,407 to 8,956.
=== Available carbohydrate === Isomaltulose is an available carbohydrate like sucrose and most other sugars or maltodextrins, in the sense that it is fully metabolised in the small intestine, and does not enter the large intestine or get excreted in urine. When eaten by humans, isomaltulose is digested completely and absorbed. Its intestinal digestion involves the enzyme isomaltase, which is located at the surface of the brush border lining the inner wall of the small intestine. This enzyme is otherwise involved in the digestion of α-1,6 linkages present in starch. The products of isomaltulose digestion are glucose and fructose, which are absorbed and enter the bloodstream. Once absorbed, the glucose and fructose follow the same metabolic pathways through the body as if they were derived from sucrose. While fructose is mostly converted to glucose or glycogen stores in the liver, glucose from the small intestine and liver is distributed via the circulatory system to different parts of the body where it serves cellular metabolism as an energy source directly or indirectly after storage as glycogen in the tissues of the body, especially in skeletal muscle.

=== Available forms === HMB is sold as an over-the-counter dietary supplement in the free acid form, β-hydroxy β-methylbutyric acid (HMB-FA), and as a monohydrated calcium salt of the conjugate base, calcium β-hydroxy β-methylbutyrate monohydrate (HMB-Ca, CaHMB). Since only a small fraction of HMB's metabolic precursor, L-leucine, is metabolized into HMB, pharmacologically active concentrations of the compound in blood plasma and muscle can only be achieved by supplementing HMB directly. A healthy adult produces approximately 0.3 grams per day, while supplemental HMB is usually taken in doses of 3–6 grams per day. HMB is sold at a cost of about US$50 per month when taken in doses of 3 grams per day. HMB is also contained in several nutritional products and medical foods marketed by Abbott Laboratories (e.g., certain formulations of Ensure and Juven), and is present in insignificant quantities in certain foods, such as alfalfa, asparagus, avocados, cauliflower, grapefruit, and catfish.
The TAAR1 full agonist RO5166017 and the high-efficacy TAAR1 partial agonist RO5256390, as well as the antipsychotic olanzapine, produced similar effects in these paradigms. Relatedly, RO5263397 produced a pattern of brain activity in rodents similar to that of antipsychotics. In addition, RO5263397 potently suppresses hyperlocomotion in dopamine transporter (DAT) knockout mice. The preceding findings suggest that TAAR1 agonists like RO5263397 have antipsychotic-like properties. In contrast to classical antipsychotics however, RO5263397 did not show extrapyramidal-like symptoms like catalepsy in mice, and instead partially prevented haloperidol-induced catalepsy, suggesting the potential for an improved tolerability profile. RO5263397 has been shown to reduce behavioral sensitization induced by cocaine in mice. Similarly, it reduces the expression but not development of conditioned place preference (CPP) by cocaine in mice. Analogously to cocaine, RO5263397 dose-dependently attenuates behavioral sensitization to methamphetamine, reduces self-administration of methamphetamine, and blocks reinstatement of methamphetamine-seeking behaviors in rodents. It also attenuates methamphetamine-induced impulsivity in rodents. In general, the drug has been found to suppress methamphetamine-, cocaine, and nicotine-induced stimulant-like and reinforcing effects in animals in vivo. Analogous findings have been made for morphine and ethanol. RO5263397 has similar effects compared to TAAR1 full agonists like RO5256390 in vivo in terms of psychostimulant modulation.
=== NMDA receptor === NMDA receptors are modulated by big dynorphin, producing effects that are antagonist-insensitive to opioid receptor blockade. Intracerebroventricular injection of big dynorphin produced memory enhancement in passive avoidance tests, enhanced locomotor activity in the open field test, and anxiolytic-like effects that were blocked by the NMDA receptor antagonist MK-801 but resistant to nor-BNI, a selective KOR antagonist. In contrast, dynorphin A and dynorphin B at similar doses produced analgesia (mediated by opioid receptors) but did not produce the NMDA-dependent memory enhancement, anxiolysis, or locomotor stimulation. Suggested mechanisms are interaction with the polyamine binding site or the NR2B subunit. The molecular basis for this atypical activity may involve the extended C-terminal region unique to big dynorphin, which contains multiple basic residues (arginine and lysine).
=== Detectors === Far-IR spectrometers commonly use pyroelectric detectors that respond to changes in temperature as the intensity of IR radiation falling on them varies. The sensitive elements in these detectors are either deuterated triglycine sulfate (DTGS) or lithium tantalate (LiTaO3). These detectors operate at ambient temperatures and provide adequate sensitivity for most routine applications. To achieve the best sensitivity the time for a scan is typically a few seconds. Cooled photoelectric detectors are employed for situations requiring higher sensitivity or faster response. Liquid nitrogen cooled mercury cadmium telluride (MCT) detectors are the most widely used in the mid-IR. With these detectors an interferogram can be measured in as little as 10 milliseconds. Uncooled indium gallium arsenide photodiodes or DTGS are the usual choices in near-IR systems. Very sensitive liquid-helium-cooled silicon or germanium bolometers are used in the far-IR where both sources and beamsplitters are inefficient.

Thymosin fraction 5 is a mixture of many peptides obtained from thymus tissue, while thymosin alpha 1 is a single defined 28-residue molecule. The two names appear together in older literature because the purified peptide was first obtained from that mixture.
The precursor, prothymosin alpha, is encoded by the PTMA gene in humans. The 28-residue peptide corresponds to the N-terminal segment released from that larger protein.
It is produced as a fragment of prothymosin alpha and is present in many tissues. Circulating concentrations are low, which makes routine measurement difficult.
Trials differ in patient population, dose schedule, background treatment, and the endpoints used to judge success. Many are small and single-center, so random variation can dominate the reported effects.
